1. Services Provided

We provide mental health services via secure telehealth, including:

• Psychiatric evaluations 
• Medication management
• Therapy services (when applicable)

Services are provided by licensed professionals.

3. Appointment Cancellations

• At least 24 hours’ notice is required for cancellations or rescheduling 
• Late cancellations or no‑shows may incur a fee

• Payment is due at the time of service unless otherwise arranged 
• Unpaid balances may result in suspension of services

1. Code of Conduct

Clients must maintain respectful, appropriate behavior. Disruptive or abusive conduct may result in termination of care.

3. Limitation of Liability

• Outcomes of treatment are not guaranteed 
• Services are provided on an individualized basis; results vary
• Align Brain Health is not liable for indirect or consequential damages

4. Medical & Emergency Disclaimer

• Services do not replace emergency or inpatient care 
• In emergencies, call 911 or go to the nearest emergency room

5. Intellectual Property

All website content, materials, and documentation are protected by intellectual property laws and may not be reproduced without permission.

6. Governing Law

These Terms are governed by the laws of the State of California.
